Statement of Need:
Second-generation antipsychotics
(SGA) produce fewer extrapyramidal side effects, a lower rate of tardive
dyskinesia, and have a broader spectrum of efficacy compared with
first-generation antipsychotics. Because of this superior profile, SGAs have
more or less replaced first-generation antipsychotics for the treatment of
schizophrenia. In addition, multiple clinical trials have now demonstrated the
efficacy of SGAs in bipolar mania and bipolar maintenance. With the recognition
that SGAs are useful in the treatment of both schizophrenia and bipolar mania
and that combining them with classic mood stabilizers such as valproate may
result in increased efficacy, the field of psychiatry is witnessing a
convergence of pharmacologic approaches to the treatment of schizophrenia and
bipolar disorder. Until the precise pathophysiology of psychotic disorders can
be elucidated, clinicians are likely to continue using blended treatments for
both schizophrenia and bipolar disorder. In this evidence-based psychCME TV
activity, the faculty will explore the current evidence for the use of SGAs in
these two psychiatric disorders.
